Pharmacy Operations, Dispensing & Medication Safety

This section describes the complete pharmaceutical care workflows in the Pharmacy Dashboard, including prescription clinical verification, barcode-assisted dispensing, dangerous controlled drug registers with dual-witness authentication, inventory replenishment, cold chain monitoring, and billing.

1. Safety Architecture & Four-Stage Lifecycle​

The Pharmacy module manages the complete prescription lifecycle from receipt through dispensing. It implements two independent safety layers:

  1. Prescribing Layer: Drug-allergy and drug-interaction advisory displayed to the Doctor at prescribing.
  2. Dispensing Layer (Enforced Safety Gate): Enforced hard stop (403 error) at dispensing that blocks the Pharmacist if a drug alert has not been acknowledged by the prescribing clinician.
  3. Four-Eyes Rule: System-enforced — the staff member who prepares the medication cannot perform the final check.

Patient Safety Checkpoint: Dispensing Hard Stop (Enforced Gate) If the linked prescription has an unacknowledged drug alert, dispensing is blocked with a hard stop (403 error). Contact the prescribing doctor immediately. The doctor must review and acknowledge the alert before dispensing can proceed. Document all communication. Never attempt to bypass this block.

2. Pharmacist Workflow — Four-Stage Lifecycle​

Stage 1: Prescription Clinical Verification​

  1. Open the Dispensing Queue. New prescriptions appear with status Pending.
  2. Confirm: patient Moana ID (check patient banner), drug name, dosage, route, frequency, and prescribing clinician.
  3. Run the drug-allergy and interaction check. If a HARD STOP alert appears, dispensing is blocked. Contact the prescribing doctor immediately.

Stage 2: Medication Preparation​

  1. Assign the verified prescription for preparation.
  2. Select the correct drug, dose form, and quantity from pharmacy stock.
  3. Print and affix direction labels with patient name, Moana ID, drug name, dose, route, and administration instructions.

Stage 3: Final Check (Four-Eyes Enforcement)​

  1. A DIFFERENT qualified Pharmacist or Senior Pharmacist opens the order.
  2. Performs the independent final check against the physical package and prescription.
  3. The system prevents the same user from completing both preparation and final check.

Stage 4: Dispensing & Patient Counselling​

  1. The final-checking Pharmacist dispenses the medication to the patient or inpatient ward.
  2. Pharmacy stock is automatically updated.
  3. A dispensing charge is automatically posted to the patient's billing account.
  4. The medication activates on the nursing Medication Administration Record (MAR).
  5. Provide and document patient medication counselling.

3. Controlled Drugs (Dangerous Drugs) Ledger​

For Schedule II / Dangerous Drugs (e.g., Morphine, Fentanyl, Pethidine, Oxycodone, Methadone):

  1. Cryptographic Witness Credential Check: A secondary witness clinician (Doctor or Registered Nurse) authenticates using a 5-minute secure cryptographic token. Dispensing without a witness fails outright.
  2. Discrepancy Investigation Gate: Any physical stock count discrepancy automatically opens an investigation. The investigation requires both a documented root cause AND a documented outcome before it can be closed (enforced at the server level).
